K21 Health Foundation awards grants to nonprofit organizations, programs and projects, focusing on our four Health Pillars: Community Culture & Wellbeing, Direct Services & Prevention, Health Access & Opportunity and Youth Development & Activity. We award grants to organizations that meet the following requirements:

  • You are a nonprofit agency with current IRS tax-exempt status and in good standing with the Indiana Secretary of State, a school system serving students of Kosciusko County or a governmental agency serving residents of Kosciusko County.

  • The project or program you are seeking funding for must benefit the residents of Kosciusko County.

  • The impact addresses our community's health and wellness needs and opportunities.

Application Process

Review our mission and what we fund.
As part of our grant application process, you will be asked how your project or program will advance K21’s mission, how it will benefit Kosciusko County residents, and which of our Health Pillars it most aligns with.

Meet with our team.
Our team would love to meet with you before you submit anything (even if we already know you)! Learning about your idea can help us guide you in the right direction.

Submit a Pre-Grant Questionnaire.
Our Pre-Grant Questionnaire, often called a Letter of Intent (LOI), has five questions to assess whether your project aligns with our funding criteria before submitting a full application. This is the first form you will complete in our grant portal.

Await notification of next steps.
Once we receive your Pre-Grant Questionnaire, our Grants Manager will review your submission and contact you to either open an application or schedule a Pre-Application Meeting. During this meeting, our Grants Manager will discuss your ideas, help you determine if your project meets K21’s requirements, and provide guidance on the type of grant to apply for and how to best position your request.

Submit an application.
Once you receive confirmation of the application phase, be sure to complete all requirements and submit by 5PM on the dates in the Grant Deadlines table below.

Grant Deadlines

K21 accepts grant requests on a quarterly basis. Pre-Grant Questionnaires and applications are due by 5PM.
