We accept grant requests quarterly across all four of our Health Pillars.

The Pre-Grant Questionnaire is a brief form, available in the grant portal, that lets you share your idea with us before starting an application. After our Grants Manager reviews your submission, we may reach out to schedule a meeting to better understand your proposal and how it promotes health and wellness for Kosciusko County residents.

Pre-Grant Questionnaires are due by 5PM on the dates listed on the How to Apply page.

Once you have submitted a Pre-Grant Questionnaire and have met with our team as needed, you will receive confirmation of the application phase.

Yes, you can submit more than one application in a single cycle, but you must fill out a Pre-Grant Questionnaire for each application you intend to submit.

Once you click the “Submit” button, you should get an email right away confirming your submission. If you don’t see an email, check your “Junk Mail” or “Spam Filter.” Make sure to add the contact to your address book so you’ll get future emails.

After the deadline, K21 staff will review the submitted applications. Once the review is complete, an appointment to discuss your application will be scheduled. All applications will then be submitted to the K21 Grant Committee, which meets quarterly to assess requests and decide on grant awards.

Typically, it takes about a month and a half from when you submit your application until the Grant Committee meets to make a decision. You will be informed of the Grant Committee’s decision a few days after their quarterly meeting.

How do I manage my grant?
Log in to the online portal to manage your grant. You will receive automated reminders from K21 through our grant portal about upcoming deadlines associated with your grant. Make sure to add the contact to your address book so you get all emails.
