Health Services Pavilion

The Kosciusko Health Services Pavilion, located at 1515 Provident Drive in Warsaw, Indiana, houses nonprofit tenants and programs that provide health services to meet the community's needs. The project began as a white paper through the Kosciusko Leadership Academy, highlighting how centrally located health and social services would increase accessibility and benefit many community residents.

K21’s Role

The Health Services Pavilion building was developed and funded by K21 Health Foundation. It is a separate entity and is not operated by K21, but we are still involved, with a K21 representative serving on the Board of Directors.
